Mission

As providers, we have become increasingly aware that chronic health issues such as obesity, asthma, diabetes, heart disease, addictions, etc. are challenging us as a health care system. The current model of health care has not been adequate to make us a healthier population.

There are many determinants of health besides biologic risk factors. These include social and environmental issues such as where one lives, what food choices one has, access to clean water and air, and education, just to name a few.

We realize that the path to personal health does not begin when a patient enters our clinic doors. Prevention is the key and it starts in our communities. It starts "UPSTREAM"

We invite anyone who might be interested in learning and exploring about the intersection of public health and medical care or in advocating for those who are less well served by the current models to join us
