Background InformationTrust for America’s Health (TFAH), in partnership with The John A. Hartford Foundation, developed the Framework for Creating Age-Friendly Public Health Systems (AFPHS). This Framework is based on 6Cs that outline six areas of age-friendly public health activities. We would like to use the wealth of resources provided by TFAH as a starting point for statewide conversation, collaboration, and shared learning, while welcoming and integrating all related efforts that improve the health and wellbeing of people living in Oregon as they age. Whether someone’s interests lie in AFPHS directly, or more broadly in chronic disease prevention, resilient communities, equity, social determinants of health, caregiving, or something else, there is a place for them in this section.
